Parents, Attestation Form is required for each child, please print, complete, sign and bring with you when you pick up on child/children packets and or Chromebook. We will have these forms printed and available for you to fill out at each campus. If you will not be picking up a Chromebook(s) for your child/children please fill out form(s), sign and email directly to the campus your child/children attend see respective campus emails.

Due to school closure (COVID-19), Vanguard Academy is providing the resources below in order to continue providing instruction to your child/children.  The electronic devices are to be used strictly for educational purposes.  All devices must be turned in on April 6, 2020.
 
Please list the name, campus and grade level of your child/children.  Initial next sign and complete the section titled Decline of Electronic Device(s).
